If you don’t see your insurance on the list, give us a call; we are actively adding new insurance enrollments.
Please be aware that although GRACE may be considered in-network, our practice may not yet appear in their system as it can take several months for new practices to be fully integrated.
As insurance coverage can be variable and change throughout the year, it is a good idea to check your individual coverage. This list is not a guarantee of coverage or insurance payment.
If you have insurance, we will always bill your insurance for medical visits and procedures first.
If you are interested in self-payment options for medical services, please contact our office for a cost estimate based on your specific imaging needs.
Spa services must be paid at time of service and are not eligible to be submitted through insurance.

As a small, local business, we partner with a third-party company called Valley Medical Management Services (VMMS) to handle our billing and insurance claims. Initial questions regarding billing, payments, and financial assistance should be directed to VMMS at (308) 234-5520. We understand how challenging and frustrating it can be to navigate insurance coverage, and we are here to support you in any way we can.
If you require financial assistance, we are here to help. Improving access to care and reducing barriers to breast health is a key part of GRACE’s mission. We offer payment plans and other options to ensure you receive the care you need.

Glossary of Terms: Navigate your insurance coverage
Insurance providers, coverage types, and individual plans vary widely. We recommend contacting your insurance provider directly to confirm your specific benefits.
Charges for diagnostic or supplemental breast imaging are often
similar across facilities. Insurance companies often determine the amount of coverage and type of cost that will apply to you based on the place of service.
We encourage you to check about your specific plan’s coverage when scheduling your exam. GRACE Breast Imaging & Medical Spa is happy to provide you the infomation you need to help you get the most accurate information about your coverage.

When deductible applies, you owe 100% of the amount that insurance allows for payment on the care you received. This amount is considered “covered”, but is applied as credit toward your deducible amount on the policy that you are responsible for paying. If medical care you receive is subject to your insurance deductible, that means your insurance will not pay any portion of the costs until you have fully satisfied your deductible amount for your policy’s benefit period. Typically the benefit period is a calendar year, but this can vary from plan to plan.

Routine annual mammograms for patients without breast symptoms. These are typically covered 100% by insurance, regardless of where you receive care.

More in-depth testing for cases where a mammogram reveals an abnormality or when breast symptoms are present. Diagnostic evaluation can include specialized mammogram pictures and/or targeted ultrasound images. Out-of-pocket costs may apply, depending on your plan’s coinsurance, copay, deductible, and out-of-pocket maximum.

Additional screening imaging with screening ultrasound, contrast enhanced mammography, or breast MRI used in the setting of dense breast tissue, genetic mutations, family history, or other reasons. Insurance coverage for supplemental imaging varies by plan and these services may involve out-of-pocket expenses depending on the specifics of the coverage.

The level of insurance coverage and type of out-of-pocket cost that you will have for services is often dependent on place of service. The place of service for GRACE is an office (specialist); the “place of service” for Goldfinch is an independent laboratory.
